Hematospermia demystified

An anxious colleague recently came into my office and closed the door. “I think I might have prostate cancer,” he said quietly. After a moment of stunned silence, I asked him about his prostate-specific antigen (PSA) level and whether he had had a biopsy. “I haven’t had a PSA test recently, and I’ve never had a biopsy,” he replied. “What makes you think that you might have prostate cancer?” I asked. “Well,” he said, “my wife and I had sex last night, and she noticed that there was blood in my semen.”

Few things alarm a man and his partner more than seeing bloody ejaculate, a condition called hematospermia, or hemospermia. It conjures fears of cancer or a sexually transmitted disease. While it’s true that hematospermia may indicate prostate cancer or another urologic disease, that’s usually not the case. In many instances, it has no apparent cause. Just as puzzling, the condition can be limited to a single episode or can occur repeatedly over several weeks or months before disappearing completely.

Doctors have been diagnosing hematospermia for centuries — even the Greek physician Hippocrates, who lived from around 460 to 377 B.C., described it. But the true prevalence of the condition remains a mystery. A busy urologist may see several patients a year with hematospermia, but it’s likely that far more cases go unnoticed by a man or his partner during intercourse. And some cases certainly go unreported by men who’d rather not find out what ails them.

Although health care professionals can’t quantify hematospermia’s prevalence, they have been increasingly able to ascribe the condition to a particular cause, thanks to better diagnostic techniques, advances in medical imaging, and some good old-fashioned sleuthing. From there, they can either prescribe medication or another treatment, or offer an anxious patient some much-needed reassurance that the condition will resolve on its own.

Among the culprits…

Historically, doctors linked hematospermia to sexual behavior. As the authors of an article in The Journal of Urology explain, patients were “warned about excessive overindulgence and prolonged sexual abstinence.” And as recently as a decade ago, most cases (up to 70% in some studies) were diagnosed as idiopathic, meaning that they had no obvious cause. The condition, researchers believed, was “benign and self-limited.” But in 2003, thanks to improved imaging techniques, a team of researchers classified only about 15% of cases as idiopathic. The rest of the cases were attributed to dozens of other factors that they grouped into six categories, as follows.

Inflammation and infection

Inflammation of any of the organs, glands, or ducts involved in the production or storage of seminal fluid can lead to hematospermia. These include the seminal vesicles, vas deferens, epididymis, prostate, and urethra (see Figure 1 below). Inflammation can be caused by irritation or trauma; stones or calcified deposits in the prostate, seminal vesicles, bladder, or urethra; and infections with viruses, bacteria, fungi, or parasites. Sexually transmitted diseases, such as herpes, gonorrhea, and chlamydia, have also been implicated in hematospermia.

Some studies have shown that inflammation and infection are the most common causes of hematospermia in younger men (the average age of hematospermia patients is 37) and up to 39% of cases over all.

Inflammation and infection, an obstruction, a tumor, vascular abnormalities, systemic factors, medical procedures, and trauma may cause bleeding that shows up in semen, a condition called hematospermia, or hemospermia. Obstruction

Obstructed ducts, such as the ejaculatory duct, and the formation of cysts in the prostate or seminal vesicles can cause hematospermia. When ducts are blocked, nearby blood vessels can dilate and rupture. An enlarged prostate, also called benign prostatic hyperplasia, pinches the urethra and can lead to hematospermia, too.

Tumors

Benign polyps and malignant tumors of the prostate, testicles, epididymis, and seminal vesicles may cause hematospermia. Men whose sole symptom is hematospermia are more likely to have prostate cancer than men who don’t have blood in their semen, but the chances are slim. A Northwestern University study of 26,126 men who underwent prostate cancer screening proves the point: among all study participants, 6.5% were diagnosed with prostate cancer. Among those who complained of hematospermia, 13.7% were diagnosed with prostate cancer. (For more details, see “Hematospermia and prostate cancer,” below.) Even though the finding is statistically significant, keep in mind that more than 86% of men with hematospermia don’t have prostate cancer. One could argue, too, that these numbers may be elevated because a screening study like this is likely to attract men at greater-than-average risk of prostate cancer.

A review of other scientific articles found 33 tumors in 931 patients with hematospermia, or 3.5%, far fewer than in the Northwestern study. Of the 33 tumors that were identified, 25 were prostate tumors. The other eight tumors were found in the seminal vesicles, testicles, and epididymis.

Vascular abnormalities

Blood vessel abnormalities in the seminal vesicles, bladder, prostate, seminal vesicles, and spermatic cord (the vas deferens and its accompanying arteries, veins, nerves, and lymphatic vessels) may be the source of bleeding.

Systemic factors

Several diseases and disorders that affect the whole body have been linked to hematospermia. These include severe hypertension (high blood pressure), a bleeding disorder called hemophilia, leukemia, and chronic liver disease.

Trauma/medical procedures

Most cases of hematospermia are probably caused, unintentionally, by medical procedures. Transrectal ultrasound prostate biopsy (TRUS-PB), which removes bits of prostate tissue to check for cancer, provokes it. The reported incidence of hematospermia following a biopsy varies between 5.1% and 89%. A recent study by Miami researchers pegged the incidence of hematospermia following TRUS-PB at 84%. On average, hematospermia lasted three and a half weeks before resolving on its own. (For more details, see “Hematospermia and TRUS-PB,” below.)

Other medical procedures, including radiation therapy, brachytherapy, transurethral resection of the prostate (for BPH), and vasectomy can bring about hematospermia, as can testicular or perineal trauma, pelvic fracture, injury during sex, and prolonged sexual abstinence.

Admittedly, reading this long list of possible causes may do little to reassure a man with bloody semen. My colleague, for example, now not only worried that he had prostate cancer, but also that he had polyps, cysts, blocked ducts, and tuberculosis, too. But if you experience hematospermia, don’t let your mind run wild. (Remember that most cases can’t be attributed to a particular cause and resolve on their own without treatment.) Instead, schedule an appointment with your doctor or urologist. If a cause can be determined, it can be treated. If no cause can be found, you’ll probably breathe a bit easier.

Detective work

Your doctor or urologist will begin by taking a detailed medical history and asking questions about your symptoms: How and when did you notice blood in your semen? Have you had a single episode of hematospermia, or has the condition been persistent? Have you had any urologic tests or a prostate biopsy recently? Have you traveled to any areas where tuberculosis is common? What medications are you taking? Have you experienced other symptoms, such as fever, unexplained weight loss, urinary problems, or pain? He or she will also ask about sexual activity.

Next, your doctor will conduct a physical exam to rule out various conditions that can cause hematospermia. He or she will take your blood pressure and temperature, feel your abdomen, examine your genitals, and perform a digital rectal exam to feel for hard spots on the prostate gland and for cysts in the seminal vesicles.

In addition, you will have a blood test, urine analysis, and urine culture. If sexually transmitted diseases are suspected, your doctor may test for them. And if there’s a chance that the blood may have come from your partner, your doctor may suggest a condom test: ejaculate is collected in a condom and then examined for blood.

Just as there is little consensus about when, or even whether, a healthy man should have a PSA test, physicians don’t always agree about when or whether patients with hematospermia should have one. Some experts say it’s necessary only in patients over age 50 unless the patient has a family history of prostate cancer. Others, including me, recommend PSA testing in all hematospermia patients over age 40 because, as I noted, it can be a sign of prostate cancer. The incidence of prostate cancer in younger men is quite low — according to the National Cancer Institute, only 0.6% of cases are diagnosed in men age 44 or younger. So, in my opinion, PSA testing before 40 probably isn’t necessary unless you have other symptoms. In an older man with a borderline-high PSA score, I would consider ordering a prostate biopsy.

Men who have persistent hematospermia, blood in the urine, or other symptoms, or who are over age 40 may have a transrectal ultrasound (TRUS) to look for abnormalities in the prostate, seminal vesicles, and other tissues. During this procedure, the doctor inserts an ultrasound probe into the rectum (see Figure 2 below). The probe emits sound waves and then “listens” for echoes as the waves bounce back off surrounding tissues (the prostate, for example).

During a transrectal ultrasound, an ultrasound probe is inserted into the rectum. The probe emits sound waves that bounce back off surrounding tissues. Depending on the strength, pitch, and direction of the reflected sound waves, a computer can create pictures of the internal anatomy.

Based on the strength, pitch, and direction of the reflected waves, a computer creates pictures of the internal anatomy (see Figure 3 below). Several studies have shown that TRUS effectively pinpoints usually benign abnormalities that can lead to hematospermia in 74% to 95% of patients. Among the findings: enlarged seminal vesicles; stones in the seminal vesicles, prostate, or ejaculatory duct; cysts; and BPH. Interestingly, no cancers were detected in these studies. Because it is so effective and minimally invasive, TRUS is the first type of imaging that should be performed.

Depending on what information TRUS yields, your doctor may recommend two other procedures: magnetic resonance imaging (MRI) and cystoscopy. Unlike TRUS, MRI can reveal bleeding in the seminal vesicles or prostate. Cystoscopy allows your doctor to examine the inside of the bladder and urethra, areas that may not show up well on MRI and TRUS, with a thin, lighted instrument called a cystoscope.

A trained eye can spot structures such as the prostate, bladder, and rectum on a computer-generated transrectal ultrasound image. In some cases, cancerous lesions may be apparent.

Case closed

The treatment for hematospermia, obviously, depends on the cause. Infection, for example, should be treated with antibiotics. Bleeding in the seminal vesicles, urethra, and prostate can be halted with an electric current. Any systemic problems, such as high blood pressure, should be controlled or treated. And if the amount of blood in the semen is slight, or in cases where there is only one or two episodes of hematospermia, a wait-and-see approach might be best. However, if hematospermia returns, check back with your doctor. Some doctors have found that treating chronic hematospermia with finasteride or dutasteride solves the problem, though no clinical trials have proven the medications to be effective in hematospermia patients. Others may prescribe antibiotics if they suspect asymptomatic prostatitis.

As for my colleague, I am happy to report that he is fine. He had several tests, including a TRUS, but we weren’t able to identify the cause of the bleeding. He did mention that he and his wife hadn’t had sex in quite a while, so the spotting that he saw may well have come from overly full seminal vesicles. Regardless, he hasn’t experienced any more episodes of hematospermia.

While hematospermia can certainly be frightening, it is a benign disorder in most patients. A quick visit with your doctor will probably reassure you that hematospermia is little more than a case of smoke without the fire.

    After 4 months of no blood in semen, it is back. No clue why. I suppose I will just have to accept it. All bloods and infection fine. No sign of anything that is causing it. I believe it is weak blood vessels in the penis that are ruptured or bleed with the slightest touch. Will update again soon. Jay

    My heart sank when I saw Ciprofloxacin didn’t work with you. I have thought I will have the same outcome but feel I need to try. I’m told that the urologists will see me again if the antibiotics prove ineffective but the GP has also told me that they tend to look on this condition as usually something not to be given too much concern. Reading the posts here, I feel that there is a lot of suffering being endured and believe, like you, that more could be done. The statistics ( see the NICE guidelines below) that the causes are usually benign but if you are over 40 there is a greater possibility of some malignancy (albeit still statistically small).

    I haven’t read anything about seminal vasculitis (and no medical person has mentioned it to me) and I’ll look it up.

    Haematospermia is the visible presence of blood in the semen. The blood appears bright red when bleeding has occurred recently and red/brown when it is old.
    In most cases, haematospermia is benign and self-limiting.
    In men younger than 40 years of age, the most common cause of haematospermia is infection.
    In men of 40 years old and older, an underlying malignancy is a more common cause than in men below the age of 40 years.
    Investigations include:
    Measurement of blood pressure.
    Culture and analysis of a mid-stream urine sample.
    Examination of testes to detect lumps, tenderness, and/or swelling.
    Digital rectal examination to assess the prostate.
    Other investigations guided by clinical findings, for example, tests for sexually transmitted infections, full blood count, coagulation screen, renal and liver function tests, and scrotal ultrasound (if there is a testicular swelling).
    Management in men younger than 40 years includes:
    Treatment of an underlying cause if identified.
    If an underlying cause is not identified, providing reassurance that a serious underlying cause is very unlikely and that the condition will likely be self-limiting.
    Referral to a urologist if there are more than ten episodes of haematospermia.
    Management in men 40 years and older includes referral to a urologist for further assessment after even one episode of haematospermia (unless the man has had a recent prostate biopsy).

    I was also put back onto Ciprofloxacin a couple of years ago, the first time around it seemed to make a difference and if it is caught early enough I think it could work. Second time around did nothing for me I’m afraid.

    41 – been having symptoms of Hematospermia for around 8 years. At first it wasn’t too much of a drama, just a bit of spotting that I noticed in my semen, over the years it has got more constant, and now its pretty much the norm. I have had pretty much every test going but nothing ‘bad’ has shown up. By bad I’m talking cancerous or similar. Through some on-line research I have convinced my self that its seminal vesculitis, as I have noticed that I also now produce less semen. We managed to conceive a child through IVF three years ago, when we tried for the second after a TESE they discovered that I am now totally infertile. I can’t help but think the two are linked. Was back at the consultant again today who is at a loss. Has referred me to someone in London who ‘may’ be able to help, although I suspect not. The only studies that I have seen that address severe Hematospermia, all come from China, certainly nothing that I can find in the UK, or anyone in the UK who treats it. Feel free to prove me wrong if anyone knows of anyone. I refuse to believe that nothing can be done. I also dont think it is as rare as we are led to believe. Just that not many men will come forward for treatment, due to our nature . Onwards and upwards.

    Brit, 70, had blood in semen for the first time approx 4 or 5 years ago. Had STD checks, prostate exam, PSA etc but nothing found. On and off problem persisted during masturbation and I noticed this was more likely if taking a low dose Aspirin which thins the blood. A few months ago put on 20mg Xarelto which is a more advanced blood thinner. Following that I get blood in urine and I said now is the time to get further checks done! But despite cystoscopy, CT, ultrasound, blood tests etc etc no underlying disease or cause can be found. The blood in urine has stopped but not the hemaspermia. One other issue that may be related is that despite sometimes having a lot of pre-cum and orgasm there is little or no ejaculatory fluid/sperm. This has made me suspicious I have retrogade ejaculation i.e. sperm being ejected into the bladder. I havent had this tested yet. Any further developments and I’ll post them here.

    I’m 58 and first noticed blood in semen when I was in my 30s. I went to the doctor, was referred to urology but a cystoscopy and prostate massage showed nothing conclusive (except that I seemed well in other respects). I had another one or two episodes of hematospermia that were short-lived. The latest episode, however, still ongoing, is the most worrying for me. I noticed a far heavier presence of blood last December. The doctor had a sample tested, but this showed no infection. I was then referred to urology, had another cystoscopy and then, a few weeks later, an ultrasound scan of my testicles. Again, nothing adverse shown. Examination of my prostate showed no enlargement and PSA levels are not elevated. I was also put on a two-week course of an antibiotic (Ciprofloxacin), which made no difference.

    Things are still the same, with some fluctuation: I thought I was getting better but then had a dramatic presence of blood once again. I am reassured that there may be nothing serious going on but like some of the men above I find myself worried about being able to engage in sexual activity. The latest advice I’ve had is to go on the antibiotic again for a 5 or 6 week period. Has anyone found that an antibiotic did the trick?

    54 and have had blood in ejaculate off and on for almost two years. No pain, but I have also had kidney stones since my 20s, and the bearable ongoing symptoms of those overlap with whatever I’m observing in my ejaculate. So sometimes I wonder about the cause of occasional minor discomfort during ejaculation. It can on occasion be very slightly painful but with no blood, so…

    At times ejaculate can appear to be 100% blood; other times 100% blood-free. The cycle is roughly: blood for several days, ‘clean’ for a few weeks.

    I now consider this to be just another thing that probably doesn’t matter that I’m unable to correlate with some other thing that I think matters. PSA, blood and urine tests indicate health, not disease.

    Interestingly, I also have this small ‘lesion’ on the side of my nose that disappears for several weeks then reappears for a week or so. I think there are things that just happen in and on us that have nothing to do with anything. We can still relax, be happy, enjoy life, and observe. For YEARS of health. When we die, God will tell us what it was and we’ll go “OH!” The End.

    Just an update from 2 posts back. Went for blood tests and everything was fine. GP is baffled. Said it’s something that will come and go. My psa test was fine. I still have brown blood in sperm. No other symptoms.

    I am happy to report that after over a year I no longer have blood in my semen. So what was the trick? Well it’s one that most you aren’t going to want to hear. I stopped drinking alcohol. Didn’t really drink much before. Like 1 beer a day and the odd late night out. I was kind of sick of drinking anyway and found it easy to stop. I have some non alcoholic beer to hold me over. Cause I love HD taste of beer. After nearly a year and a half of Doctors telling me it’s no big deal and not understanding that it is a big deal (no sex). I’m cured. That’s my story a d I hope it helps others.

    Hi guys, 2017 June I got my first episode of blood in the semen. Went to doctor, had MRI, ultrasound and nothing showed up. Got antibiotics and it cleared up. Then in 2018 it returns. Going for psa test and fbc this week. My doctor said it’s very common but nobody discusses it in public. He thinks it’s a weak blood vessel which overtime has decided to pack it in. Imagine your clutch in the car and then think of all the overuse your penis had. Don’t worry or lose sleep, just know that we are all dealing with it. Jay

    Hi I am 83… had a turps 8 yrs back so all my orgasms are dry… thats not the problem . I piss a lot of blood if I exercise or do some gardening or have sex … I notice that when my urine is clear the stream is weak, but if there is blood then I urinate as if I was again ten years old very forcefully a lot of blood and clots but no pain… anybody here can relate to that? I also notice that the blood always comes out first followed by the urine, which make me rule out kidneys and bladder . I take no medicines at all dont eat red meat or drink coffee would like to here from anyone who can think where the problem is.

    James here , 49 Years Old ,6’2 , 305 lbs . My first experience with blood in semen was about 5 years ago while masterbating . It appeared to be 50/50 . 50 percent semen , and 50 percent blood . And it scared the hell out of me . I went straight to the ER that night and they checked my urine….it was fine . They checked my blood , and it too was fine. They sent me home with a good bill of health , But I did so very confused , and very concerned .

    A few weeks later I go see a Urologist . He does the finger up the rectum to check for an enlarged prostate…it’s fine he says . He then checks my testicles….states they seem to be fine too. He then basically tells me not to worry about it , and that it happens sometimes. Again I’m sent home confused, and concerned.

    Well , Ever since my first experience , I have had it happen again a few times each year, and its pretty nervewracking each time . But I kept reminding myself what the ER Doctor , and the Urologist had told me….” It happens”. So I just tried to ignore it when it did happen.

    August 2017 , I had a friend tell me about his PSA test , and how it saved him from some problems . He anxiously advises me to have a PSA done . Two weeks later the blood in the semen happens again . This time I google ” Blood In Semen ” and begin to scare the bejesus out of myself. I’m reading that it could be prostate Cancer……OMG!!! My friend just recommended that I get a PSA test !! So now, Now that I’ve just scared the shit out of myself , I run down to the ER and again explain the blood in my semen . This ER doctor runs a urine test…just fine. Runs a blood test…just fine . and gives me a 10 supply of Antibiotics JUST IN CASE . He’s just about to release me when I ask him if he could take a PSA blood test . He agrees but tells me that it would take 5 days . So what I say . I’m game. So I take the blood test , and he sends me home.

    Few days later the PSA score comes back and it’s 0.4 . Which is supposedly good.

    So now I go see a Urologist , He has me go get an ultra sound on my testicles , does a blood test , and checks my hormone levels.

    Ultrasound came back with a tiny cyst on left testicle , blood test was fine , and hormone levels were fine.
    He has me redo the ultra sound 4 months later. Comes back the same small cyst on left testi , but no growth or change.

    So at this point it’s happening every month now ( Blood in semen) Its like it clears up SLIGHTLY for maybe a week out of the month , then bloody the rest of the month.

    It’s nerve wracking to say the least.

    Last week I had a strange electric like pain on my left side it was really quick pain , but unlike any pain Ive ever had Before, and I’ve had 4 spinal surgeries ( 1 Thoracic 1 Cervical , And 2 Lumbar). Anyway the pain came and went like shockwaves for about 15 minutes, then just went away. A few days later I notice a small amount of blood on the tip of my penis when I urinated. So once Again I run off to the ER runs a urine test on me and yes there was blood in urine . No infection though.

    Now for this , after talking with a nurse , I believe that I may have passed a kidney stone . But who knows.

    4 days later I’m in my Urologists office and he says its customary that when blood is found in the urine to do a cystoscopy . Not at all thrilled with that , but hey he’s the doc right ? I tell him that he has to KNOCK ME OUT for the procedure though.

    So I’m set for this Monday . Nervous, worried , and scaring the hell out of myself by GOOGLING things I probably shouldn’t be GOOGLING . HA!

    Maybe I’ll find out whats causing the blood in my semen . At least that’s my hope. But for all my fellow brothers out there that is experiencing this too , Hang in there , You’re not alone , through these scary , worrisome times.

    I have had this problem since I was 20 years old and I’m now 45. It would go away pretty quickly in the beginning but in my forties has been very persistent and is affecting my relationships not to mention my psyche. Over the years I’ve been to five or six urologist I’ve ended up in the ER couple times not being able to pass blood clots I’ve had CAT scans MRIS cythoscope at least six seven times. And as always they find nothing just say it’s like a nosebleed and you can just keep having sex no biggie. But if I fight through it ,the wound or whatever is damaged inside seems to get worse and worse and blood literally can drip out of my penis. Part of me has given up on trying to dig for answers because urologist don’t seem to care enough to look a little deeper. Even though I’ve had the million-dollar work up as they say. I still feel like they’re missing something just can’t put my finger on it. But hey I’m not a doctor it’s just my body I guess. So most you guys are experiencing the same thing I always have been the past 20 years I would definitely always get checked at least a few times to make sure it’s not cancer but for the most part unfortunately it is something you have to live with and can get worse as you get older. I’m still praying for a cure I can’t have sex the way Id like to have sex. I’ve controlled my blood pressure I quit casting I take vitamins drink lots of water somewhat good and nothing is working. Only weird thing is bleeding mostly curve through sex not always so much through masturbation which makes me think it’s not prostitute it’s somewhere in the urethra or some other area with trauma. If anybody finds a miracle cure please share

    Left 2017 cumming blood and entered 2018 just the same. Have read a lot about this topic and apparently something that may help is losing weight, lowering blood pressure (no salt, lipids, sugar), drinking a lot of water, very hot baths in that area… so I’ve been working on these for 2 months now and so far ok. I feel lighter too. Must say that my results were all fine so no physical malfunction. Will post again if this works… or not

    Hi guys am so glad and thank every one of you who have posted your problems on here .. I got blood in my sperm last week and have been very depressed about it thinking am going to die .. But after finding this I feel a lot lot better now .I thought only me that had it . It seems it affects lots of men . Am 61 and to my horror seen blood in my sperm after sex .. I not very brave person when it comes to this kinda thing .. But feel much better as you guys have it as well . Am going to doc on Friday .. Thanks all you people and all the best .

    To make a long story short I’m 65 years old, retired, and in very good health. At about the time blood appeared in my semen I also started having high blood pressure for some unknown reason.     I am an active 78 yr/0. I dont cycle on a regular basis, but when I do it is usually for several hours over bumpy roads. This seems to correlate well with outbreaks of blood in my ejaculate. I have never found a really comfortable bike seat. The front of the seat is always hitting right between my anus and the cojones. I cant think of any solution other than stopping riding a bike. Does this resonate with anyone else?

    First of all, really glad I’m not alone. I don’t know why there isn’t much online about this, but so glad I found you guys and this information. This post should be the first thing that pops up when you search for it.

    Started three years ago. Midway through masturbating I started bleeding profusely. Thought I was literally gonna die. Went to the doctor and he wasn’t too worried like I was. Gave me antibiotics and something to help the bleeding. Throughout the next couple weeks I would experience bleeding even just doing normal things like at work. Once while squatting down to pick something up.

    After about two weeks it stopped. Completely gone.

    Fast forward three years later and I’m at work and I suddenly feel wet like I peed myself. Look down and there’s blood. Schedule appointment again with urologist. Same thing. Antibiotics and something for the bleeding. Three weeks pass and I. Still bleeding profusely while masturbating. As soon as I start it just keeps going and going. I can’t even orgasm.

    Decided to go to a different doctor. Had blood tests. Had ultrasounds of the kidneys bladder and prostate. All good. Again antibiotics and now anti-inflammatory meds. Thinks that I have seminal vesiculitis and or prostatitis.

    Somethings to consider. I’ve had a lot of sexual partners, but practice safe sex, so no STDs came back. Im in really good health, being a physical trainer, I have to be, so that cant be it. although my diet isn’t all that great. Some vitamin defiency might be it. Some of you mentioned caffeine also. I don’t drink any coffee but I do take caffeine based pre-work outs and caffeine pills at work. Compared to people I know it’s not excessive in my opinion. Roughly 300mg. Creatine as well. 5g a day. I drink about a gallon of water a day.

    My next step is probably to go completely off any supplement. Stop masturbating along with any sexual activity. I’m having a trans rectal ultrasound in the next week so we shall see what that shows. Probably all normal.

    To summary for those looking for somebody in a similar situation:

    -28 year old 160 lbs
    -single, masturbate about 3 times a week.
    -very sexually active
    -okay diet. No excess of anything like red meat or whatever.
    -drink lots of water.
    -not on any medication and no history of prostate cancer
    -i take caffeine supplements and creatine
    -high intensity workouts from time to time
    -have blood before and after orgasm…but not mixed with semen.
    -have blood discharge sometimes spontaneously during the day.
    -sometimes bleed while sleeping. I’m guessing since we tend to get erections while we sleep.
    -not pain whatsoever. Only real time I have felt anything was when I felt a blood clot come out. No pain just discomfort.
    -have bled while having a bowel movement. I’m guessing it stimulates the prostate or seminal vesical while doing that.

    Final thoughts: I mean it sucks. Really bad. Messes with your personality and attitude. I’ve had to wear pads for three weeks now to feel a little less paranoid about bleeding in public infront of people. It has saved me twice now. I have had to pretty much ignore any sexual thought that comes to mind and have almost pretty much stopped approaching women for that reason.i feel as though if I get even a little aroused I will bleed. I’m really hoping this isn’t going to have to be something I have to deal with for the rest of my life. Even if it’s just on and off.

    Hey guys, 19yo here. I’ve had this problem since this June. I went to the urologist about a month ago and she told me to take it easy, and if not, come back. Which brings me here, tonight, because after about a month and a half of being just fine, we’re back at it again.
    The first round I had was in June, where my loads would have about half blood and half semen in it. Close to the end of it (the first time) it got bloody. So much so, that upon inspection I could feel how the blood was dehydrating in my hand, with no sperm to be found. Magically, it then went away for about a week, even though I hadn’t slowed my roll, keeping to my regular of handling myself about once a day.
    During that time, I was trying to explain to the docs I was seeing that it was fully blood red, not just a little bit or a minor thing to worry about. There is no pain that goes along with it, the blood’s either there or it isn’t, and if so, a clot and bloody urine come after. Then everything’s fine till I repeat the process again. But the reason I’m typing is because I have a few causes I have a few causes I think contribute to this, and I wanted to consult you guys and see if there’s a common thread here.
    – I’ve been a smoker since 17, and at most I’ve smoked a pack a day
    – I had a serious case of incopresis since about 4, and it followed me till I was about 16. I’m still sometimes irregular.
    – I can feel a bit of a numbness (but no pain) in my left testicle in comparison to my right. Numbness may not be the best word but there is a difference that I can tell, and before this ever happening I was fine.
    – I drink about 2 energy drinks a day to keep up at work, I have been for the last 2 weeks and my water consumption in comparison is l o w .
    – This is the third time I’ve been shocked to see and hear a clot hitting the bowl, and this problem has seemed to go away twice.
    – I’m pretty vigorous during masturbation and honestly, I think it could be a serious cause as to how it started.
    – When I was looking down at my shaft (while erect) a time or two since this all started, I noticed I had like this strange depression on the left side of my shaft at the base, the right side was fine. I haven’t noticed it since but it could still be there.
    – I have relatively low blood pressure on an average day, even though I’m at 210lbs at 5’9 and a smoker.
    – About a month before this happened, I was tubing in a local river with a friend of mine, I really have no idea if the water was tainted or if this also happened to him: We both know we don’t talk about this with friends over for dinner, do we reader?

    Anyways gents, that’s all I can really say about this. If there’s any advice or knowledge anyone has about this, please man, help a brother out. As it stands, my urologist told me if this happened again, she’d have to investigate with a urethra-cam, so that’s my next (and hopefully last) trip there. I have no idea where this is coming from, how to treat this, and after reading these posts literally I’m just asking more questions. Hopefully somebody has something, and if you do it’s appreciated as much as you already can imagine.

    My doctor said my cause of blood in my semen was a burst blood vessel in the prostate during stimulation. I also pass blood during a bowel movement probably the reason its hard to recover permanently. My doctor said he could laser zap the blood vessels to seal with surgery but may not be 100% cure. I think the cause of finding a cure in today medical field for this problem has more cons then pros. Maybe in 5 years technology can improve and this problem can be solved with a reasonably cost solution. We need to pressure the medical field to do research in this field.

    Taking aspirin daily caused me to have hematospermia and it went away and or became less frequent. I have also noticed that chronic constipation (IBS-C) can put pressure on the prostate and cause it to bleed. I’ve had this off and on for 10 years or more but everything checks out O.k. so I just don’t freak out and it will usually resolve in a while. Just gets more common with age.

    37 y/o American guy living in Italy.

    I’m not too concerned and am going to keep pressing forward to find a fix, but this stuff is just getting old!

    April 2016 – First noticed blood when my wife and I had sex, was very faint and pinkish. I didn’t ejaculate for a couple days. Tried again, and it was slightly worse. My immediate self-prognosis is “I’m dying”. Very scary stuff.

    May 16 – Visited PCP who did prostate exam, no enlargement or hard spots. Performed sperm culture(showed slight sign of infection) was prescribed an antibiotic, 30 days. Then referred me to an Italian Urologist.

    June 16 – Visited Italian Urologist, received another prostate exam, no enlargement, asked a few questions and prescribed me Daflon 500(high powered vitamin) and another Vitamin (forget what it was called, but was a blueberry extract). Took both for 45 days, no change. They were supposed to strengthen venous walls.

    August 16 – Bloody ejaculate still persisting. Usually would last about 3-4 days every other week or so.
    Made appointment with PCP to refer me to an American Urologist (this is where the fun began, HAHA)

    Sep 16 – Repeat prostate exam, received Transrectal Ultrasound, Hormone blood tests, MRI on Pituitary gland.
    *Ultrasound showed normal images, Hormone test showed slightly low testosterone levels, Pituitary MRI showed normal, but a couple of unidentified orbs or something, said it could literally be an old injury, so not really concerned*

    Bloody semen still persisted.

    March 17 – Went back to American Urologist – Received MRI on prostate(Normal), PSA(Normal range), repeat hormone test(unchanged), cystoscopy(normal, but SUCKED!!) also had repeat sperm culture, showed no sign of infection.

    So here I am today. Trying to figure this out.

    In 2015 I was deployed in Afghanistan for 6 months and noticed that my volume and consistency of ejaculate was getting less and less quality. This was probably from the excessive amount of masturbation (sorry just being honest), so I wrote it off as no big deal. Also, full disclosure, I was chronic masturbator my entire adult life. I have/had an addiction to it. Am working on ways and am having success in getting past that.

    Looking back, I think there was possibly an infection that went untreated and could’ve cause scarring in the ejaculatory duct?!?!

    I don’t know if the excessive masturbation has had anything to do with this, but I would not doubt at all that they are somewhat correlated.

    Going to pursue abstinence and holistic approaches for a permanent fix to this(if there is one). If I find a solution, ill surely post it.

    Glad I’m not alone in this with you guys.

    I had pink ejaculate during a period of two months last year when I was 58, a very distressing period. It was usually followed by a few drops of blood in the urine. My urologist had my semen and urine tested for cancer cells, which both came up clean. An ultrasound and CATScan didn’t show any tumors. The ultrasound found a kidney stone but the CATScan disproved it, something my urologist had learned from experience. PSA was 2.7 and digital prostate exam was normal though enlarged with age. Since the bloody discharge was still going on after all the tests proved normal, my urologist still went ahead with the cystoscopy that had been authorized initially. That was an unpleasant experience to say the least, and bleeding was encountered around the prostate so that he couldn’t continue to examine the bladder. That was followed by two days of very bloody, painful urination, but after that there was no longer blood in any discharge. The diagnosis was that it was a vascular issue related to the prostate area, and I have been taking 5 mg Finasteride for the past year, starting daily, but after several months of no bleeding down to 3 times a week. Possible causes? Daily bike riding, and I also remember a couple of very strained, firm bowel movements that preceded the episodes. A year later I still am relieved to have clear discharges. I’m grateful for sites like this, WEB MD, etc. since my first thought when I saw the bloody discharge was “I’m dying!”

    I am a 42 yr old white male. I have sex regularly (2 times per week) I am coming up on my two year anniversary since noticing blood in my semen. I had pain in my left testicle, scrotum and abdomen. Went to the doctor and was given a prostate exam and gave a blood sample for a PSA test. PSA came back normal and the doctor found noting remarkable about my prostate. My urologist is a family friend so I get more access to him than others do. I was put on antibiotics and it cleared up.     My husbands first episode was blood we noticed in the semen after intercourse about 5 years ago.. Then the next several times he had to pee he would pee clots. Seemed if he took round of Cipro he would improve and sustain from sex for couple weeks. This would occur for a couple months periodically every year. Although it did skip a year in 2014 I believe.     Hi, thank you first of all for your article. Can you refer to a specialist in this area? I have been having persistent hematospermia off and on for the past 15 years…and I’m only just 30 years old.

    Over the years I’ve had al kinds of tests done on me, including prostate exams, cystoscopies and MRI-scans. The most recent MRI-scan (Q1 2016) revealed an inflamed prostate, of which the inflammation just refuses to die. The blood vessels in my prostate appear to be very weak and easy to rupture. The longest I have lived without an episode of hematospermia has been 2 years, but that was way back in 2004-2005. Since then, the longest amount of time without it has been close to 6 months. I have been taking Avodart for a couple of years, but it does not heal my prostate indefinitely. It does reduce the inflammation a little bit, but those blood vessels still rupture – just not as easily when I stop taking Avodart. When I don’t use Avodart it may go well for approx 3 months on average, before the inflammation lights up again.

    This hematospermia has been ruining my sex life for years and I don’t know what to do. My girlfriend’s genitals also get very irritated by my use of Avodart, but without it, the chance of seeing blood with my semen increases immensely. I live in the Netherlands and no specialist over here seems to be able to do something about it. They have all always ruled out prostate cancer, but I’m not so sure about that. Why else would my case be so persistent?

    I don’t care where in the world I have to go, if there is a specialist that can help me out with this, I will make use of that specialist. Is there anyone you can refer me to?

By the time I got int to see the urologist it cleared up. I also have. a history of severe rectal pain following ejaculation. January 2013 I underwent bilatteral inguinal hernia repair 2 months later a vasectomy. Following the hernia repair ejaculation felt restricted. Urologist suggested it was likely from the flomax and finesteride I was on. I discontinued them with no improvement. Fall 2015 I had bloody ejaculate again (it looked like 100% blood just like the first time). This time my urine was tested and there was trace of blood in urine. Urologist ordered abdominal CT scan and scoped into my bladder. No cause found. Then in May 2016 my wife again pointed out the bloody semen. My doctor tested urine but no blood this time. By the time I saw the urologist it had stopped. Then a month later, this past week, the blood returned. Labs show anemia with good iron level and PSA 0.5. I am waiting to see if insurance will approve an MRI.

    Any thoughts on cause for these problems and next steps?

      I gues that we need to live with it for the rest of our lives. I’m only 26 but have it for over 1 year and a few months from time to time. My urologist doesn’t see any problem that may cause it.

    What a relief to find this, reassuring post. I’m 43 & had my first episodes around 2 years ago, had a variety of tests which found a bug in my urine. Also had ultrasound scan, no other issues found & it cleared up in its own, without medication.
    Fast forward to years & I just had another episode, albeit a lot more blood this time. It’s had me up all night worrying.
    However having read this & based on my last experience, despite each episode being hugely alarming, the general consensus is that it’s likely to be caused by a low grade infection/inflammation. Therefore, my first plan of attack will be to take a VitC & Zinc boost (just as I would when I get a cold), reduce caffeine & red meat from my diet (not that I consume much) but more importantly I will start to drink a lot more water, as this is something I tend not to do. I sincerely hope that this combined with regular ejaculation will flush away whatever the cause may be, without medication.
    If that fails, of course, it’ll be another trip to the GP for antibiotics & tests to rule out anything more serious. Although, I hope this strategy works because if this is a common recurrence, I don’t really want to have to visit the GP every time it happens.
    Will report back with progress. All the best & thank you for sharing a very personal issue.

    I’m 49 years old. I have been seeing blood in my seamen for the past 8 months. I had a PSA test done and it showed to be ok. I urinate a lot everyday. Which is most likely because of a UTI or enlarged prostate. Out of my own personal research on the net, cranberry juice has been highly recommended.

    For the first three days I had cranberry juice twice a day. Thereafter I only had 1 glass of cranberry juice everyday. My bleeding in seamen stopped completely after about 3-4 weeks . I started to decrease it use to once a day. All seem to be fine. I then stopped it completely. The bleeding started again recently.

    So presently I’m on it again everyday to see if it will stop again. It was good to know that it stopped at first. But It was a bit worry some to know that I may have to drink cranberry juice every day.

    It may not work for all.. But it may work for some… good luck guys….

    I’m have this problem last week ago June 1, 2016, i saw on my semen there was a blood clot and after the ejaculation i almost pee blood and some blood clot. i think i got this problem when me and my live in partner accidentally have sex that she still got menstruation, because we thought she’s menstruation already finish when we got sexual contact. please help what should i do? it make me depressed :'( can’t think properly what to do with this. i’m taking antibiotics for almost a week.     Thank you so much for the educational information it has helped to relieve many fears that went through my head.
    I am 60 this year so naturally my first assumption was that crap I’m falling apart.
    I am quite healthy and in great shape I stayed quite active with a job in construction doing 50 to 60 hours a week with no problem.
    I go to the VA for doctorING and I can’t say enough good things about my primary care team they are taking excellent care of me.
    I’ve made an appointment to research all the factors and let them do their thing determining what the true cause is.
    But one thing I didnt notice in your article was something simple that the tel-care nurse mentioned to me when I was thinking my appointments, overuse or over indulgence in sex with my partner I’m curious what the community would say , is that a possibility ?
    In any event thank you again for steering me in the right direction.

    First of all, thanks to all for sharing your experiences. Only two days ago I had a rough ejaculation, like it just had trouble coming out after masturbating, then hours later I urinated and a nice size clot of blood came out but the urine was clear as water. Two days later I was brave enough to masturbate and sure enough, blood, but once again clear urine. Naturally, in the middle of the night I searched online and found this helpful site. Like everyone else, the big C is the first thought and I really freaked but you’ve really helped settle down my worries. I work long hours sitting and have started drinking coffee. I’ve had urinary tract infections, the last in 2008. I also on ocassion have severe bouts of gout. I’ve lost weight, which also helped my blood pressure. Now out of nowhere I ejaculate blood. I’m 55 and in good health so this really has me puzzled. I’ll first try laying off the coffee and in a week or so try “things” out. I sure hope it goes away. My wife and I rarely have sex and to tell her this would cause even more concern, despite the fact that she’s been my only sexual partner, I’m sure she’d wonder if I had another.     I’m 41 years old and have had hemospermia for around 6 years now. Like most of the others here, when it first happened I was freaked out and went to the Dr. I didn’t like the fact that the Dr wasn’t concerned about it, and I didn’t think he was very thorough. I went to two more Dr’s and the third was a little better than the first two. I had many different test ran with no apparent reason or cause for the blood. It’s been several years now since I’ve seen a Dr, and the problem still lies. Prostate Cancer has taken the lives of my Grandfather and my Father. Should I be more concerned with this because of my family history, and my age? I do plan on trying to cut out the red meat, and start drinking more water to see if that helps.

    For around the the last three to four years I have had intermittent periods of bright red blood in my seminal fluid when ejaculating. Usually these episodes have lasted no longer than two to three days but at present I am encountering yet another episode and this time the problem has been ongoing for around three weeks continuously. I have spoke with my doctor but this has only been over the telephone and he advised me to produce only a urine sample for analysis which I have done. The result of the sample showed traces of blood, protein and some other matter which I cannot recall the name of but otherwise the test was given the all clear. I am still fairly concerned as the problem is showing no sign of let up and I do have dull ache in my left testicle and groin. Around one year ago I had the problem examined by a doctor in Belgium where I work and the diagnoses was an infection of the prostate. Am I worrying for nothing and will the matter resolve itself? Can you please offer me some advice on what to do if anything.

    After dealing with this for almost 4 years, with no help from Doctors. I read a comment that someone had cut out all caffeine and excess sugars and loaded up on water intake. 1 week of this, and clean as a whistle. so stressed all these years and a simple fix. Hope it helps some of you others, guess it was just a reoccurring prostate infection from not drinking enough water. Too much Tea and energy drinks did me in. Good luck

    50 years. Have had this condition 1/2 my life. It used to come and go but now it’s pretty much here to stay most times I have sex, though only rarely when it’s just myself in the room.

    I have good news for anyone who ever needed a catheter because of the clots. For me, pissing right after sex only works sometimes because it seems I am already too late the clot has formed. Anyway, I discovered this a few years ago – you can piss into a bottle lying on your back. Gravity pulls the clot down and away from the opening of the bladder. You may only empty half your bladder each time, but keep it up, drink water, and worst case you’ll pass the clot before midday the next day.

    The worst part of the blood used to be the trips to the hospital and the take-home catheter. Now that’s a thing of the past, no matter how big the clot.

    But I am scheduled to have a TURP in 7 days, as a kind of gamble to fix this problem that is not going away. Any advice? Should I do it or not?

      Bill from April 2016 is correct. Had several clotting issues after erections/sex. Last one blocked me up and I couldn’t pee from the clot. So after reading what he wrote I payed in the shower and the pee came right out. Then most of the clot, then stood up and peed normal again. No catheter for me! Hopefully will be fine. Last time this took two rounds of cipro to get rid of. Just finished round one. My doc says once you have the first infection they come more frequently.

    39 yo-
    -I’ve had the issue of bleeding during ejaculation and had blood in my urine followed by blood a clot. All occurring ONLY after sex or masturbation. The urine was very dark and the clot was so big, that at times it was the size of a quarter and sometimes I couldn’t pass it.
    -I’ve had this 3 times within the past 8 years. Each time lasting from 6 months to a year. All three times it was cleared up by taking sulfameth/trimethoprim 800/160 mg tb. The only issue with this is that the antibiotic didn’t clear it up immediately. After taking ST immediately I had no results. It was when I waited 6 month and changed my diet and took it again, did the symptoms go away. I changed my diet to a ton of water, vegetables, no red meat, very little alcohol, no canned food. I also tried to eat as clean and organic as possible.
    I also took proscar for the 6 months as well as Uni-Pros-Trol. Good Luck. I hope this helps.

    24 year old male here. So I’ve had about a total of 10 ejaculations that were bloody (80%ish). I have never had it happen during sex with a woman. The only times it happens is when I “milk” my prostate with a dildo and ejaculate. The bloody ejaculation is followed by bloody urine drops (sometimes very small clots) for about 15 min and then I’m blood free again. my questions are:
    1. am I damaging my prostate?
    2. Is it my seminal vesicles?
    3 is it a capillary issue surrounding both structures?
    4. Is it my diet? (I drink moderate levels of caffeine and eat red meat once a day on most days) no more than 10oz of steak a day five days week
    5. Why do we not have a clear answer about zinc or this saw palmetto business?

    Clifford toulmin – I too went through ALL the tests and all came back fine (except slight calcification in my prostate)! After dealing with hermatospermia for roughly 5 years, the doctor wanted to put me on Proscar! I did a lot of research on Proscar before I decided to start taking it and instead tried Saw Palmetto. I took (2) 360MG of Saw Palmetto twice a day and after about 3 days into Saw Palmetto, the blood went completely away! I ended up taking it for about 3 months and never again have I had any blood! This stuff is a miracle!! I did however notice a little shrinkage down there, but no more blood to report! I’m glad I decided to try SP before taking Proscar. I think I can contribute my issue (slightly enlarged prostate) to taking high doses of Creatine years ago. I can not praise how well SP worked for me! I will of course do yearly prostate exams / PSA checks to make sure all is well going forward. Again the only side effect was the slight shrinkage.

    Guys, be very careful about how you interpret hermatospermia. I had it in early 2015, went to a urologist to investigate it, and it turned out that I had advanced prostate cancer. My PSA was over 14 and biopsies had a very high Gleason score. I also have two metastices in my spine and ischium. I’m having a prostatectomy soon and have been controlling the tumors with hormone therapy.

    Just had red blood seman. Just like every body I thought it was wife. Until I showered and noticed red drop. 49 don’t drink smoke but am dieting. Two time this week blood red… Going see urologist had the exam 4 years ago and al was fine. Seem to be quite common without cancer or am I wrong. I drink a lot of coffe and hardly any water.

    I’m a 36 year old male, I had the same problem, blood in my semen, mainly when I had an erection for a long time before an orgasim, a lot of blood, kind of back flows to my bladder then if I don’t urinate immediately it starts to clot and becomes a nightmare to pass, comes and goes, had a bad one a month ago and could t urinate, ended up in a &me and having s catheter fitted the width of a hose pipe, had a ft scan and waiting a camera down my penis, I’m worried sick, had my prostrate and bloods checked and they were all fine?

    I’m only 36 surely this should t be happening to me this often, any ideas anyone please help?

    I was bleeding 90% blood I took antibiotics cipro. STD also did STD TEST IT WAS CLEAR

    i am a 60 yr old man who initially had blood in his semen about 8 months ago. my uroligist told me to wait a week before ejaculating and it would stop. i was also put on two supplements to lower my psa. i have had recurring bouts of my psa acting like a yo yo. it went from 39 to 11 to 7 to 3 as i have had numerous bouts of prostititus. i was given antibiotics and my psa has always gone down . it is normal , about 3 now. i have been tested for blood in my urine, had a sonogram and the finger up the ass, everything is ok and normal
    the only thing is the blood in my semen has come back and my urologist is not worried. i have no idea what to do. should i take finesteride or another drug or just live with the blood?

    Hi All, I am 24 and currently experiencing this so called hematospermia. I was really frightened at first that’s why I came out to this page, but after reading all the comments it makes me feel comfortable. I am actively doing an ejaculation regularly , sometimes it took 2-3 times a day. So I am guessing it has been the main cause of it. I hope that it wouldn’t turn out so bad for me. I will also avoid ejaculating regularly to see if it’s going to work also refrain from eating red meat. I’ll try to think positive!!

    hi iam 23 year old I have been deeling with this problem maybe a year ago. when I first saw this is scared the crap out of me so I started working out runing drinking water and a diet. It stopped ,last week I was having sex with my girl I ejaculated and blood 95%.. I was scared it’s been 2 months and it’s still going on . I went o the doctor and too antibiotics 1week ..     Hi, I’ve been experiencing blood in my ejaculate for about 2 years, it’s never really gone away or returned to normal: constantly watery. I’ve had all the tests from the Urologist, including an MRI scan and cystoscopy but all inconclusive. Around Mar 15 is was diagnosed with a DVT and have been taking Rivaroxaban (Xarelto) since then so, I imagine, the blood will continue whilst on these tablets (blood thinners). Over the last week, it has got really bad and is practically 100% blood. I’m also a competitive cyclist and ride about 10-15 hours a week. I don’t eat red meat but do drink excessive amounts of coffee and I’m pretty sure that I don’t drink anywhere near enough water.

    Thanks for the potential caffeine / dehydration correlation which I’ll try from tomorrow and see what happens; clearly, I don’t want to stop cycling (!!).

    Naturally, a bit distressing and sex life virtually non-existent; any advice associated with Xarelto greatly received.

The doc first said give it a week and it will go by itself. It did not. The he gave Cifran which I took for 10 days. The colour of ejaculate turned from pink to dirty yellow-brown…and then became normal and thick as before…in about 15 days from taking Cifran. But now after a month only, it has come back to haunt..have got pink ejaculate again and it is watery again..don’t know how to get rid of this problem permanently…

    My situation is almost identical to Gary’s above. I am also 54, and started noticing considerable blood in my semen around 18 months ago. It has continued on and off since. Sometimes I can go almost a month without an episode, and then it will return out of the blue. Usually when I get it, the semen is more liquid than solid. I have got to the point where I am frightened if I don’t ejaculate very day, because generally, if I miss a day, I am more likely to experience the blood. I too have had PSA tests (normal), DRE and even an MRI scan – all showing no abnormalities. I am hoping that the cause is vascular and that some small vessel breaks occasionally, however, when I do get an episode it worries me and spoils my day. I don’t usually drink coffee or eat red meat, but the situation has coincided with me cycling to work and back daily (about 6 miles in all). This could just be a coincidence though. I am so grateful for this website and up-to-date comments from people in very similar positions as myself.

    So glad to find this site. I am a 54yr old and have had this issue for two years. Did the rounds of anti biotics, CT scan,Cystoscopy, and more blood tests than I can recall. Everything comes back normal, without any real answers. Go back every six months for additional urine, blood test, but am getting discouraged that nothing seems to change. I have never heard of the coffee, red meat correlation, but am going to give that a try. thanks for the posts. It is a little more reassuring to hear of others with the issue.

    I have had this issue off and on for almost three years. Started with just a faint pink tinge occasionally. Then sometimes a brownish rusty color but would just come and go. Now I notice even when there doesn’t appear to be blood present my ejaculations are much less volume and very thin and clearish. I might see it clear up for a few days, then bright red with a lot of blood then less the next several times back to brownish tinge (like old blood) then back to a bright red with a lot of fresh blood. I am 49, pretty good health, been on blood thinners for almost a year due to a stoke caused by a clor that got thru a hole in my heart. I had the hole fixed in April and have been on aspirin full dose daily since. My issues have been the worst since I’ve been on the aspirin (more so than when I was on Xarelto). I have had urine tests. Ultra sounds and several digital prostrate exams as well as psa test with no conclusions. I chickened out on the uroscopy as there is no blood on my urine only in my semen. I like others am frustrated and nervous but do find a little solace in reading others stories as I too see I’m not alone. I do drink coffee and enjoy a fair amount of red meat. I am going to reduce my intake of these items and see if that helps. Thanks everyone for sharing.

    So I have been fighting blood in my semen for about 5 years now – on and off. I’m 39 years old and I work out pretty regularly and am really fit. I have gone to a handful of urologists and non were able to help or give me any indication of the root cause. I recently had a CT Scan done and all checked out normal. except some minor deposit in my prostate (nothing that alarmed the doctor)?? My last thing to get checked out is my bladder for any sort of issues (Cystoscopy). I suspect this will also come back ok (I hope!). I drink about 4-6 cups of coffee a day and take pre-workout supplements (caffine based) when I workout. After reading many of the post on this forum, I decided to switch to decaff and layoff the pre-workout. I also stopped eating redmeat and so far I have not had 1 episode of blood. I suspect I have finally found my culprit!! Why is this soo hard for urologists to figure out! Any way, I hope this helps someone out there!

    I am 55 have been dealing with this issue for over 25 years. I have similar experiences as Jimmy (11/25/14 post) and Andrew (12/26/14 post)– blood/clotting/spraying all over the place when passing the clot. I have had several urologists over the years and endless test and they found nothing. One once said some people are prone to bloody noses– some people get this.

    The only suggestion they have had was to take Avadart, which shrinks the prostate. I have not yet taken it because of the potential side effects. Up until recently I would have blood and clotting after ejaculation– and no rhyme or reason as to when. About every year and a half or two years– and I would abstain from sex for about 2-3 weeks and all would be fine again.In the past year it has been more frequent– about every 4 months. Also just started getting blood and clotting even after just getting aroused- without ejaculating. Just spent 5 hours in the hospital last week because a clot prevented me from urinating. For me it was always important to urinate immediately after it happens– as it is critically important to pass the clot. I have ended up in the emergency room 3-4 times over the years and not being able to urinate is pretty stressful and dangerous.

    I think this forum is great for all of us dealing with this issue to learn what people are trying and the results. Based on what I read I will be trying to drink more water, eat less red meat, and exercise more. I’ll post with any new updates.

    I have had this off and on for about 10 years. It comes and goes and varies in how much blood is there. I was diagnosed with a mulerian duct cyst. I had it drained once, which was not much fun. The specialist I saw afterwards basically said masturbate/have sex more and it will go away. Which to be fair, it does. I’ve always been able to link these episodes to some pretty aggressive BJs that I’ve received or aggressive pounding as I top.     I’m 54 and have had problems since 49 when l was admitted to emergency dept. with AUR. Have had catheter in and out with increasing frequency and has been in now for almost 18 months! Have had blood in ejaculate for last couple of years and a recent biopsy detected cancer, but it is very small. My urologist is more concerned about getting the catheter out of me. Like everyone else here l was freaked out about blood in ejaculate but urologists couldn’t tell me why, but I have noticed a pattern. I usually get blood after eating and drinking badly particularly beer, red wine and coffee as well as fatty food. My PSA went from 5.6 in 2009 to 31 in late 2013 which scared me so much l gave up alcohol. By the time of my next PSA it had fallen to 15, and with healthy eating and supplements it is now 10.5, so diet plays a big part Also l have had chronic constipation and IBS for a least a decade. I have read a few books on the subject and scanned many weblogs. My theory is the prostate sits very close to the sigmoid colon. If you have shit (toxic sludge) sitting close to the prostate for any length of time, its no wonder the prostate swells. The bleeding is probably due to stones or crystals formed by the body as an emergency measure to deal with the toxic sludge. Also the pressure of shitting causes it to bleed. Anyone with prostate problems must read The Natural Prostate Cure by Roger Mason. Avoid crippling surgery and a painful death and improve your diet. Meat and dairy foods with high fat levels are the cause of prostate cancer. Milk has 35 hormones that stimulate cancer growth, Professor Jane Plant in the U.K. has written 2 excellent books on prostate and breast cancer which are related. Professor Plant put her breast cancer into remission in the early 80s by giving up meat and dairy, check it out.

    I’ve had this for the first time twice lately in my 40 years. Can’t say exactly what caused it and what is causing it for you people out there and in fact there could be combined factors causing this both for you and myself. However in my case, after first submitting samples of my sperm to my urologist, it was found on both cases that I had bacteria in it(source of which being either shit or tooth cavity). So the likelihood of this bacteria causing the bleeding as a result of inflammation of the prostate or seminal vesicle seems the likely source of blood in my semen (red color) and either blood or bacteria or white blood cells in the semen (brownish color). However these may be secondary factors, hard to say, and I have still yet to have my sperm tested (by PCR) for Ureaplasma, mycoplasma or fungi. Haven’t ruled out to much cycling either as being the cause. Some doctors and specialists argue that having bacteria in our semen is not the cause of Prostatitis or hematospermia but if it is, I would advice you all to get blow jobs with condoms as I probably acquired this bacteria from those sources. So first thing you need to do is to give a semen sample when your body has been free of antibios for at least three weeks, stop cycling, you also you need to ejaculate frequently to stop any buildup of semen and bacteria. I was also put on a course of tablets that got my blood flowing there as my doctor said it may be caused by a congestion of blood there. Also said I need to sit less and exercise more (walk)On both cases he cured it for me. I’m going to have to stay away from women for a while though to see if they really are the cause of this.     Never had any blood in my semen but was diagnosed years ago with micro hematuria, tiny tiny amounts of blood in the urine. All testing was inconclusive. Probably still have it.

    The other day my female friend went overboard and I wound up with blood in my semen, I mean bright red!! Urinated about 15 minutes later and all was fine.     I am 64 and I have been seeing blood in my Semen constantly over the past seven years so far. It started in the first two years as brown clots but, in the following five years or so, I see my semen ejaculation starting normal then, towards the end of the ejaculation, the last few drops of ejaculate are reddish (almost pure fresh blood). I tried both abstaining and having more sex but the blood persisted. Two years ago, I have two PSA tests at 5 but also I had two normal prostate digital exams. Ultrasound shows that my prostate is very much enlarged. However, my urine stream intensity fluctuates. It is frequently weak but it gushes and reaches maximum flow immediately after sex. In fact, after sex, I can urinate so vigorously just as when I was so young and more healthy at 20. Therefore, as a layman, I reasoned that if I had prostate tumor, the urine flow rate (intensity) should not fluctuate because a tumor is expected to cause permanent restriction to urine flow. This is why I am reluctant to do prostate biopsy just because my PSA is 5, especially given the fact (according to research) my enlarged prostate condition itself can also be the reason for this high my high PSA. In addition, biopsies are inaccurate and, hence, inconclusive. So, I am living with this “blood in Semen” condition and intend to do NOTHING about it! Who knows, it might still go away by itself after all, but I refuse to plunge in inconclusive diagnostics which can do more harm than good!

    It appears that when this occurs the medical establishment does not seem too concerned, but to those of us that experience this it is extremely troubling…I have experienced blood in semen for three years…it first started as slight discoloration then progressed to a full stream of blood… now it comes and goes in various amounts of blood…many exams and tests conclude no physical reason for the occurrence…I am 58 years old in excellent health and take no medications for any conditions…I do recall that before this ever happens I did take an erectile drug and soon thereafter this condition started but no doctor has indicated this was the cause of the problem..I can get erections regularly and masterbate often and have noticed that even when little of no blood is present my semen is not near in consistency and thickness that I had in my younger days…I am assuming that as I age my semen was suppose to get thin and watery not thick and creamy like it used to be…sure wish there was a way to talk about this with others

    Guys I found a correlation between. Red meat consumption and this issue. Consuming large amounts of fatty red meat as causes it (for me) and being dehydrated or taking stuff like caffeine (coffee) which can dehydrate you intensifies the issue. Euric acid must be accumulating in that region.

    Eating only poultry and fish, drinking lots of water seems to resolve things for me. Sucks because I love hamburgers.

    Glad I am not alone in this…. and do need to get it checked out, just in case. That being said, I’ve been living with it for 14 years now… just turned 41…it’s an on again off again thing with me.. no rhyme or reason to it. Luckily there is no pain, just an “odd” feeling before ejaculating, and the sometimes very large clot to pee out after. Unfortunately, when it happens it’s like really blood red, not brown or yellow or lumps..just blood. Can barely tell it’s semen. SO unsettling and annoying. I’m single and have to try to explain this before sex…. how the hell do you explain this to a person you haven’t slept with yet?? Not exactly a turn-on. As if dating isn’t hard enough already. So just wanted to add my two cents that we are out there and are dealing with it too…you aren’t alone. I really need to look into treatment options…at least they are out there now.     I had a cystoscopy in Oct. 2013. Three weeks later my ejaculate was bright green with threads of blood. Doctor told me it was “normal” and would go away. By December 2013 I was ejaculating blood. I have had back to back months with strong antibiotics in the Spring of 2014. MRI, Ultrasound and another cystoscopy (different doctor) and can’t find the problem. At this point the question isn’t “blood in the ejaculate”, its is there any semen in the blood. At wits end, need to find a specialist who can fix this.

    No Evidence for zinc whatsoever or selenium or Vitamin E!
    The SELECT trial was a randomized controlled study designed to evaluate vitamin E, selenium, and the two in combination in the prevention of prostate cancer. Unfortunately, this study failed to show a benefit from either supplement. Many people were critical of the choice of vitamin E and selenium; these agents were chosen because they had been used in the studies that formed the basis for the SELECT trial.

    Now, an update of this study has shown that these agents actually harmed some men. Men with high levels of toenail selenium upon entering the study and who were randomly assigned to receive selenium (either with or without vitamin E) had a 91% increase in high-grade prostate cancer. In addition, men with low selenium levels who received vitamin E alone had a significantly increased risk for total, low-grade, and high-grade prostate cancer.

    These findings have two important implications. First, the public needs to recognize that it is false to assume herbs, vitamins, and supplements cannot cause harm. Second, this study again illustrates the importance of properly testing supplements in randomized trials rather than making conclusions from epidemiologic or uncontrolled case studies.

    I am 50 years old. I have had blood in semen for over 2 years. Sometimes even an erotic dream (without ejaculating) leads to blood in urine. Few brief periods were it stopped for a short time. I also get clots in my urine after ejaculation, and sometimes it is so difficult to poss that I must wait until the following day to develope enough pressure to get the urine out. Often when urine does finally come out, it’s extremely messy because of spray from the pressure buildup. It’s awful. Never mind the uncomfortably of bloody seamen with your partner, blood on sheets…
    I have had every test done except for MRI. told by two different urologists, nothing they can do. Hard to believe there is nothing that can help.

      as for me I started noticing blood at the age of 40, after a vigorous sex, then I saw blood clot inside the condom, and some drops of blood on my thighs dripping from my foreskin. I could not feel any pain at the time, until after some hours a sharp pain as I pass urine.
      I attributed the problem to the use of a condom, holding too much pressure in the urethra.

    I’m 84. I note here that guys some years older have reasonably successful orgasms. I had a fifteen year period of total sexual abstinence for religious reasons when I was younger, which I now regret. After struggling to hit the spot, I just gave up. Went to Yoga. Astanga. This morning two pink spots show on my sheets and a trial of my first drops of urine on a white napkin showed pink. My next urination was clear. Hoping its simple BPH and not an increase in cancer. Have an appt. with my radiologist in 2 weeks and will talk about all of it. I need to know if if a return of cancer would show up in recent cat and bone scans I had. If not cancer, is there any remedy besides my one FLOMAX one half hour after dinner. Just one makes me dizzy in Yoga or do I have to bite the bullet and now take two a day or go back to finasteride? I had the green light about nine years ago, before the cancer diagnosis; , but now the old prostate is growing. If just more BPH can the green light be done a second time. Also are there other measures for BPH considering my medical history? Please don’t let the answer be more cancer, more hormonal shots and more radiation. I had forgotten I had those problems and I’m still working long hours a day at what I do. Thanks men.

    I for the last three weeks have been passing blood on ejaculation, it started as spotting, then watery blood, its now 90% blood. I have been to the doctors, he gave me finger up the arse treatment, tested blood pressure, urine test for blood which was clear. i’m 55 and type 2 diabetic he sent me to see a specialist at the hospital who gave me the anal finger treatment again where he confidently stated I have an enlarged prostate and he believes a condition called Prostatitis. he has given me a 28 day course of antibiotics which I prey works. just had sex with my wife, not intercourse but she gave me a hand job and the condition is the same, all blood. Very scary, just got to wait and see what happens after tablets finished. going private if it does not work with tablets…. I am very worried as is the wife, looked into prostatitis and its says everywhere its very painful, well, I have had no pain so I have concern over diagnosis, they either don’t no what’s up or they like fingering peoples arse holes, my advice is get as many opinions as you can and don’t sit about listening to so called experts, keep moving forward at all times making as much noise as possible.. after all they are not the ones with the worries and the blood coming out of their dicks,,, do your doctors head in at all times that’s what we pay for in out tax and health stamps after all.

    been dealing with this on and off, for the past 4 years.
    Never once has there been blood during masturbation. Ever!!!
    However it is now to the point that regular intercourse will result in bloody semen. Which very very quickly clots up.
    I have to be basically full of water prior to intercourse. Then immediately go pee ASAP, after intercourse. If not it will clot up. Very painful to pass several clots. Sometimes taking the whole day. I once had to go to the emergency dept to have a catheter inserted to relieve the pressure followed by passing of clots.
    I have not had any injuries or accidents, good PSA results, regular prostate exams, biopsy samples taken of my prostate ( extremely painful ) and no cancerous tissues.
    My urologist is not concerned, but this is very taxing.

    thank you for this very informative article, also to user eric. lately i’v bin gaining wait and i drink a lot of 3in1 coffee to be specific and yes sometimes i feel dizzy which makes me think i have a high blood pressure problem. i started noticing my hematospermia last 2012 while masturbating. panicked, i rush to see a GP and gave me sum antibiotics, and honestly i did not believe her, i surf the internet instead, luckily i found answers like this article did. after 2weeks of abstinence blood is gone. then this august 2014 after having sex with my gf i see a lot of blood again. worried so i surf the net again and found this. and what eric said might also apply to me. i hve no other symptoms aside from the blood.

    The bloody event occurred three days ago to this 87 year-old man. It was the result of masturbation as my 80 year-old wife suffers from severe depression and is not interested in sex anymore but I am. The ejaculate was a steady very bright red. However I noticed just fifteen minutes later that upon urination my urine was crystal clear and has been ever since. Once before at least three year ago I experienced the same event took place and my urologist told me not to worry that it very well probably was a capillary that momentarily burst whithin the system. So I didn’t worry. I might add that this same urologist has me taking the drug flomax to protect my prostate and it seems to be doing a good job as all recent tests have revealed only a small enlargement in the organ. I have not sought a doctor’s opinion for this latest occurrence and don’t intend to unless the next ejaculation is a repeat. I don’t think it will be.

    I was 21 when I had my initial episode. I’d spent most of the night in a dorm lobby attempting to woo a woman I was particularly excited about. I went to the men’s room, and found myself unable to urinate except for a trickle of blood. I spent the rest of the night in the infirmary, where they poured as much water down me as possible without my teeth swimming, cleared my colon, and enabled me to find a comfortable position from which I could pass the clots formed from bleeding–well, we never could determine where the source of the bleeding was, but it definitely went into my bladder that night. I spent the next day in the hospital, undergoing a number of inconclusive tests. Eventually, they let me go with a “seems clear now.” After that, I had intermittent hematospermia. *For the next 29 years.* That brings us to *tonight,* boys. We’ve never been able to determine the source–and some docs just told me not to be too concerned. It’s not abnormalities in the prostate; that much I know. Hell, for a while I did work for the local medical schools as a practice patient and underwent rectal exams–I had to be examined at the beginning of the cycle just to be certain that I wasn’t myself an abnormal patient, and I wasn’t.

    After nearly three decades of this–on and off–I’ve concluded that my case is probably in the “vascular irregularity” category. It’s most often linked to a particularly turgid erection, and if I sleep on my stomach, I almost always pass a clot the following morning (since most men get an erection in their sleep, I figure I’m no different, and laying on said erection no doubt places extra pressure on it). If there is going to be blood in my semen, I can frequently tell, because my erection feels subtly different–however, this test is not absolute. Numerous times, I thought I would have an episode, but things were all clear.

    My approach to it nowadays is relatively simple. I make sure to stay hydrated if sexual activity seems imminent. In addition to hydration being good for you, I’ve come to learn that if there is blood in my semen, I will inevitably have to pass some blood in my urine, and if it has time to clot, better to have more urine behind it than less. I’ve never again had to pass as copious amounts of blood in my urine as I did during that first episode; not by a long sight. After an episode, I’ll normally try to avoid sex for several days or a couple of weeks, but there’s really no rhyme or reason. Sometimes I could have sex within 24 hours and show no symptoms. Other times, two, three weeks later, with no activity in the meantime, and I’ll still show blood. I do not sleep on my stomach.

    By the way, I have never had any of the other symptoms mentioned in this article–back pain, weight loss, what have you. Nor am I engaging in frequent, acrobatic, strenuous, or high-impact sexual activity.

      Experienced pretty much the same as you, with a bout of bad hematospermia (not just spotting, but full on red) and then followed by red urine with a clot. What I’ve figured for myself though is that if I try to “pressurize” the ejaculation, perhaps by squeezing a little tighter, the blood can retrograde into the bladder, explaining the bloody urine and clot. Since coming to that conclusion, I’ve stopped getting bloody urine and clot, just the hematospermia.

      Another revelation lately is that if you are constipated and/or have fairly hard stool, the stool while passing can put pressure on the prostate and cause it to bleed as well, especially when there’s some damage to begin with (i.e. during a bout of hematospermia). That would explain the initial bit of red dribble at the start of urination followed by normal urine that someone mentioned. Staying hydrated and having a good amount of dietary fibre should help.

    I am 50y/o. I had never taken medication in my life untill three weeks ago when I was prescribed a triple dose for ulcers. After the 7 day course i noticed blood in my poo just once. Today I notice my ejaculation is 90% blood. I am scared and i now imagining pain in my pipes

    Guys. My bout with it was BAD. 99% blood came out when I ejacualted for 6 months. I figured mine out. High intake of red meats. I assume that the uric acids built up way to much and it started crystallizing down there. Lay off the red meats and control your intake of coffee, sugars and dairy. Drink tons of water too. Simple. Rest & heal easy.

        My advice is to let your loved ones know. They will help you out and care for you. If you haven’t already, go to the emergency and see a urologist. They will test your blood, urine, test for infection and check your bladder, kidneys and prostate.

        Noticed that you just posted your comment within the last 48 hrs. I had this problem too. I’m 31 now and it happened 3-4 years ago. It scared the shit out of me. I thought that the devil had somehow entered my body! J/k. I went to urgent Care and they looked at a urine sample under a microscope (blood was not visible with naked eye in my urine). Doc diagnosed me with prostatitis. He gave me something like 10 days worth of antibiotics and it cleared me right up. Haven’t had any reoccurances since then. It doesn’t hurt going to a doc to see if there is maybe something more severe going on than maybe some inflammation or a rupture somewhere. Best regards. ~Ed

    I am an 85-yr old male. Recently I have seen a strong whitish ejaculate followed by a slightly weaker light brownish linear discharge. No other symptoms. Urination greatly improved after ejaculation. I have no yet reported this to my family doctor.
    Recurrent prostatitis years ago, treated with antibiotics, has not returned.
    Otherwise, I have frequent bleeding trouble(gums) possibly related to 3.1 Protime from daily dose of 2.5 mg warfarin prescribed on account of atrial flutter. No flutter symptoms in about 10 years. Minor non-clot stroke experienced months ago. A CABG 4X 12 years ago.
    Diagnosed with DDD a few years ago, weight lifting limited.
    BP usually under 120/80(systolic sometimes under 90)and HR either about 75 or about 45. Height 5’9″ down from 6,” weight about 160 down about 27 from all time high.

    I first noticed my haematospermia in November 2013,at first it was intermittent though more recently has become the norm with discoloration at first ranging from pinkish to brownish red. Its now quite fluidic and watery and comprised only of pre-ejaculate with a complete absence of semen. There is no pain and have had psa test and ultrasound which revealed nothing obvious. Next port of call is a cystoscopy.I’m deeply distressed by both loss of libido and a normal sex life.

I have been thru every test and the urologist says nothing wrong but some calculi in prostate and he cant help me any further. what about massage of transrectal heat rotating magnetic field? An article says it works but then the trail goes dead – cant find anything more. do i just have to live with this the rest of my life until it kills me?

    I am a 60yo who hasn’t had sex in about 2 years. I have masturbated on occasion, but not anywhere near as often as I would have sex, if I were married. I have suffered a few bouts of hematospermia over the last 6 months – mostly caramel colored semen. Today I had a first time experience with frank blood in the semen. There were discrete spots – large enough to be seen as spots – as large as grains of rice, and very discrete. No sensation of pain. Just the blood.

    I know, go see a doctor. But do you have any gut feelings?

    I am 35 y/o, experiencing blood clots in semen for the last 4 months n it is continued. I had n Mri ,cystocoy was done ‘ they hv found slight enlargement of left seminal vesicle wid different fluid levels with some calci on prostate. In cystoscopy they found little erosion on prostate .
    I was takin cipro then leflox wid max flow but no gain still havin blood clots.
    Really worried now.
    Doc says it jus coz of inflamation of sem vesicles. They cant do much abt it ‘ But what is the treatment?

    I have had recurring hematospermia for 3 years, am 60 y/o, have seen a urologist and had an MRI and a couple rounds of abx.

    The first time I had hematospermia a combination of abx and prostate squeezing (yikes!) indicated infection and resolved it. Later bouts were not due to any apparent infection and the MD essentially said he had no idea.

    Now I am having a strong smelling urine for over three weeks and am rushing to my PCP. My question is, do you know any urologists who specialize in hematospermia?

    Perhaps I should have gotten another opinion previously, but now I’m alarmed.Thanks very much if you can reply. If not, I understand and again thank you for the information.

    Had first episode heamatospermia during an infection nearly 2 years ago. Had xrays / cystoscopy / ultra sound testicles. Informed that it must have been infection. No problems for 15 months then unfortunately 3 times this year. Approx 3 month gaps. Now stating 2 weeks of antibiotics.
