Consider this Harvard finding before you toast the New Year

We've reported before that heavy drinking is tied to an increased risk for developing dementia. A Harvard-led study published online Sept. 27, 2019, by JAMA Network Open suggests that the risk is even worse if you have mild cognitive impairment (MCI). Researchers analyzed the data of more than 3,000 dementia-free adults (average age 78, some with MCI) who reported their alcohol use and underwent psychological testing for six years. People with MCI who drank heavily (more than 14 drinks per week) had a 72% higher risk for dementia than people who drank less than one drink per week. However, people without MCI who drank moderately (seven to 14 drinks per week) didn't appear to have an increased risk for dementia, and even seemed to have a 37% lower risk for dementia compared with people who consumed less than one drink per week. The bottom line: "Our results suggest that moderation is the healthiest approach if you drink. And if you have mild cognitive impairment, you may want to reconsider drinking alcohol at all," says Dr. Kenneth Mukamal, a study author and researcher with the Harvard T.H. Chan School of Public Health.

Blood test may find early signs of Alzheimer's

A new study found that a simple blood test can detect beta-amyloid protein buildup in a person's brain years before Alzheimer's disease symptoms appear. The results were published online Aug. 1, 2019, by the journal Neurology.

High amounts of beta-amyloid can clump together and form plaques on the brain, which is strongly associated with Alzheimer's disease. Other research has found that amyloid plaques can appear as early as 20 years before the first sign of Alzheimer's symptoms, such as cognitive decline and memory loss.

Study links certain medications to a higher risk of dementia

But some experts say limitations of the study raise questions about the results.

Could one of your regular pills raise your risk of dementia? A study published online June 24 by JAMA Internal Medicine found that people over age 55 who took a daily dose of certain common drugs for approximately three years had a 50% higher risk of developing dementia.

The medicines in question are in a category known as anticholinergics, which are used to treat conditions as diverse as depression, urinary incontinence, allergies, and Parkinson's disease. The study's authors say the findings should prompt doctors to use caution or potentially even avoid prescribing certain medications to people over age 45 on a long-term basis if there are other options available.

Gingivitis (gum disease) has been linked to a higher risk of heart disease, but a recent study says that the bacteria that cause gingivitis also may be connected to Alzheimer's disease. The study was published Jan. 23, 2019, in Science Advances.

Scientists have previously found that this species of bacteria, called Porphyromonas gingivalis, can move from the mouth to the brain. Once in the brain, the bacteria release enzymes called gingipains that can destroy nerve cells, which in turn can lead to memory loss and eventually Alzheimer's.

The World Health Organization has some advice for people who want to reduce their risk of dementia — pay attention to your overall health. Specifically, the organization's new guidelines recommend regular exercise, maintaining a healthy weight, regulating blood sugar levels, and keeping cholesterol within recommended limits.

Experts also recommend giving up unhealthy habits, including drinking and smoking. In short, strategies that you already know are good for your heart and overall health are also good for your brain. Preventing dementia is a focus because it has become a growing health problem, which affects some 50 million people around the world. The WHO estimates that nearly 10 million people each year develop the condition.

Skip vitamins, focus on lifestyle to avoid dementia

Vitamins and supplements won't help stave off dementia, but a healthy lifestyle might, suggest new guidelines released May 19, 2019, by the World Health Organization (WHO). The WHO warns that the number of new dementia cases around the world — currently 10 million per year — is set to triple by 2050. While there's no cure for any kind of dementia (such as Alzheimer's disease and vascular dementia), the WHO says it may be possible to delay the onset of the disease or slow its progression. The key: managing modifiable risks, such as chronic disease and unhealthy habits. The guidelines recommend that you keep your weight, cholesterol, blood pressure, and blood sugar under control; get lots of exercise; and eat a Mediterranean-style diet (which emphasizes olive oil, fruits, vegetables, nuts, and fish; minimizes red meats and processed meats; and includes a moderate amount of cheese and wine). The WHO also advises that you don't smoke and you avoid harmful use of alcohol (no more than one drink per day for women, no more than two drinks for men). But don't count on supplements to help you stave off dementia. The WHO says there's no evidence that vitamin B, vitamin E, multivitamins, or fish oil supplements help reduce the risk for dementia. The agency recommends against using supplements as a means to ward off cognitive decline.

Get the facts about memory loss

Many conditions can affect your memory and thinking.

Dementia, Alzheimer's disease, mild cognitive impairment. These terms are often mentioned together to describe age-related memory loss. While they are linked in some ways, they are not necessarily the same, and they often affect people differently.

"Even though memory loss is more common as you age, it is not part of normal aging," says Dr. Gad Marshall, associate medical director of the Center for Alzheimer Research and Treatment at Harvard-affiliated Brigham and Women's Hospital. "Either sudden or gradual persistent changes in memory loss and mental lapses can be signs of more serious conditions."

Poor sense of smell may predict risk of death in older adults

In the journals

Previous studies have shown a link between loss of smell among older adults and risk of death within a short period — often five years or less. Now, a report published online May 21, 2019, by Annals of Internal Medicine confirmed this increased risk of death over more than a decade, as well as identifying the leading causes of death.

Researchers gave smell identification tests to 2,289 adults ages 71 to 82 (about half of whom were men). They found that those who scored low on the smell test had a 46% higher risk of dying within 10 years, and 30% within 13 years, compared with those who had a stronger sense of smell.

So how do you keep your brain healthy, stay cognitively fit, and build your cognitive reserve? It's easier for some people than for others. And though genetics set the stage for your brain health, you can do something to improve your brain health and cognitive fitness.

First it is important to remember that you need a healthy body to have a healthy brain. Therefore, ensuring your brain health depends upon regularly seeing your doctor, following her or his recommendations, and managing any health conditions you have.
