Torn knee cartilage

Q. I am 72 years old and in good health. I've had left knee pain on and off for almost six months, nothing too severe, but it aches if I walk more than a mile or so, and I sometimes get sharp pain on the stairs. My doctor sent me for an MRI of both knees; it showed "mild to moderate osteoarthritis" in both knees and a torn meniscus in my right knee, which feels fine. I think I know what to do about my painful knee — but what should I do for the torn meniscus in my "good" knee?

A. Before we get to your interesting questions, some background information may be helpful.

Ask the doctor: How does hot pepper cream work to relieve pain?

Q. I have pain from osteoarthritis in both knees. I'm curious about the cream made from a substance in hot peppers. How does it relieve pain?

A. You're referring to capsaicin, the substance in chili peppers that gives them their hot taste. Capsaicin is an ingredient in many over-the-counter topical pain-relief preparations, which include creams, gels, lotions, patches, and sticks. When first applied, topical capsaicin causes a burning sensation. This sensation lessens within a few minutes, and also over time with repeated applications. There are few, if any, systemic side effects.

Painkillers pose problems for people with heart disease

Millions of people take nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen (Advil, Motrin and others), naproxen (Aleve, Naprosyn, and others), and celecoxib (Celebrex) to relieve pain and inflammation. During the last few years, researchers have raised concerns that taking these drugs often may be hard on the heart as well. The latest study, published in the July 2011 issue of the American Journal of Medicine, suggests that regular use of NSAIDs poses a special problem for people who already have heart disease, boosting their chances of having a heart attack or stroke. This research doesn’t mean that people with high blood pressure and heart disease should stop taking NSAIDs, especially if they are used to ease pain from a chronic condition like arthritis. But it may make sense to try an alternative first.

Taking the pain out of runner’s knee

Patellofemoral pain syndrome, also known as runner’s knee, makes it painful to walk up and down stairs, get out of the car, and, of course, run. It happens when the kneecap doesn’t run smoothly up and down its track—a groove called the trochlea. Although anyone can get patellofemoral pain syndrome, it is more common in women than men—especially in mid-life women who’ve been running for many years. Strengthening the quadriceps (thigh) muscles and stretching the iliotibial band, connective tissue that runs from the knee to the hip, can help, as can cutting back on exercises or movements that put repetitive force on the knees.

Gaining awareness about arthritis and its prevention

Many Americans are remarkably unaware and uninformed about arthritis, a disease that affects about one of every five U.S. adults. Arthritis runs under the public’s radar because it isn’t a killer like heart disease and cancer. Yet it can take a huge toll on the quality of life through the pain and problems it causes. Walking is one of the simplest, least expensive, and most effective things you can do for your health. It strengthens bones, tunes up the cardiovascular system, and, psychologically, can clear a cluttered mind. Hundreds of studies have documented its benefits. 1. Gait speed

As we get older, the stroll that was once a walk in the park may get difficult for any number of reasons: angina, arthritis, bad balance, failing vision. In the latter decades, walking becomes as much an indicator of health as a promoter of it.

Heart Beat: Rheumatoid arthritis should heighten heart awareness

Rheumatoid arthritis begins in the joints, but it doesn't end there. The inflammation that causes swelling and pain in fingers, wrists, knees, and other joints can also affect the heart. A large study from Sweden suggests that a new diagnosis of rheumatoid arthritis should get you thinking about your heart, too.

In a group of 7,500 men and women with new rheumatoid arthritis whose health was followed for up to 12 years, more people than expected had a heart attack, developed chest pain with activity or stress (angina), or needed a procedure to open or bypass a blocked heart artery within five years of their diagnosis (Journal of Internal Medicine, December 2010).

What to do when health problems or medical treatments thwart your love life

Health problems, or treatments for them, sometimes thwart sexual desire and sexual function. There may not be a quick fix for health-related sexual problems, but there are things you can do to enjoy your love life while taking care of the rest of your health.
